The practice offers private pay, Medicaid, and commercial insurance options. Cost should never be a surprise: before testing begins, you'll know what the evaluation involves and what it will cost you.

Private pay

Private-pay evaluations are billed as a flat, all-inclusive fee based on the type and scope of evaluation — intake, testing, scoring, feedback session, and the written report included.

Medicaid

• Some plans require a physician's referral or prior authorization before testing is covered — checking with your plan before scheduling can save weeks.
